Interactive Forms and Web Surveys

Interactive Forms

Many departments on campus have an interactive form on their sites. For instance, Career Services has an online form for students to sign-up for their Clark Career Exploration Program (CCEP) and Academic Advising has an online form for first-year students to provide some introductory information to their faculty advisors. Forms can be simple or complicated but all forms must be really well thought out and all possible scenarios must be taken into account or the form may not work the way you expected it to. Also, we have a responsibility to protect responder's personal data. Clark cannot and will not collect social security numbers or other important indentification information via web forms.

Creating an Online Form

Web Content Managers cannot create forms on their own but must get help from the Web Development Team, Chuck Wyatt and Tracy Axelson. The best way to get a form created is to put your thoughts about the form into an email including the goal or what it is you are trying to accomplish with this form and send that with an attached prototype of the form in Word Document form to both Chuck and Tracy

Web Surveys

Another tool that ITS uses fairly often is a web server based product for conducting research or opinion surveys. This products is used very frequently by faculty, students and graduate students for a variety of research projects and by administrators for opinion surveys and the like.

Requests for academic uses of the tool should go through the Academic Technologist for your department.
