Here you will find documents, forms, and articles of interest to you as you prepare for your arrival at Clark University during the last few weeks of August. Admitted students should revisit this sight periodically for any updated forms.

The following items must be returned to IDCE by April 15, 2010 (or as indicated on your acceptance letter):

Response Form (Word Doc / PDF) and your $400 non-refundable deposit (send this form to the Graduate Admissions Office).

Bio Form (Word Doc / PDF) (send this form to the Marketing and Publications Office).

Important Immunization Information

Massachusetts State Law requires all full-time students who enroll in a Massachusetts public or private college or university to be immunized against Diphtheria, Tetanus, Measles, Mumps, and Rubella, Hepatitis B and **Meningitis. In addition, students must complete a tuberculosis risk assessment and be tested for exposure to the disease, if required.

Students who have not fulfilled the state mandated health immunization requirements will not be permitted to register for classes. For new students, an immunization record must be fully completed, signed by a physician, high school nurse or other health provider, or a student can attach a school or military immunization record to Clark's immunization record. There are 3 forms attached to this mailing. They must be completed and returned to your department or program no later than July 15, 2009.

Students are cleared for registration only after a Health Services nurse has reviewed a student's records and determines them to be in compliance with Massachusetts state law.

Checklist for Immunizations:

Is the immunization record complete? Please have this completed prior to arriving on campus to avoid additional charges and delay of registration.  Below is a checklist to insure you complete all the necessary requirements prior to your arrival.

  • Have you received a tetanus booster in the last ten years?
  • Have you received two (2) doses of MMR given at least 1 month apart and after your first birthday?
  • Have you received three doses of Hepatitis B given 0, 1 and 6 months apart?
  • **Have you completed the meningococcal (meningitis) vaccine requirement? Please read the attached information sheet completely.  If you decide you do not need the vaccine you must submit the signed waiver on the second sheet of the document.  If you receive the vaccine, have your healthcare provider indicate the date and type of vaccine on the Immunization Record sheet.
  • Have you completed the TB risk questionnaire?  If you answered yes to any of the questions, you must be tested for TB and provide the results on the Immunization Record sheet.

All students should make arrangements for any needed immunizations with medical practitioners in their home country or state to avoid charges for immunizations and registration holds after arriving at Clark. Health Insurance

The Commonwealth of Massachusetts mandates that each public or independent institution of higher education in Massachusetts require all full-time and three-quarter time students enrolled the institution to participate in a qualifying student health insurance program or in a health insurance program with comparable coverage. 

During 2009-10, Clark University will offer a student health insurance plan through Blue Cross Blue Shield of Massachusetts. The annual price of the plan for 2009-10 for a student is $1,257. Please read the Frequently Asked Questions about the Health Insurance plan where you can learn how to enroll in this plan or how to complete a waiver if you already have coverage.

For GISDE students, you will want to plan to be here prior to the GISDE Special Workshop held August 17 through August 21.
